Our health system encompasses two hospital locations in Robbinsdale and Maple Grove as well as a network of 23 clinics which includes 13 primary clinics, 6 specialty clinics, 4 urgent care/urgency centers and emergency care offerings covering five counties. Our Robbinsdale Hospital, established in 1954, is a 385-bed facility recognized as the top Level 1 Trauma center for 25 years, as well as serving as a Level II pediatric trauma center. Our Maple Grove Hospital was established in 2009, is a 134-bed facility recognized as a top hospital in the state for Women and Children Care, with a Level III NICU, and is the largest Family Birth Center in the state (~5,000 deliveries per year and over 60,000 babies delivered). About this position
Diabetes Educators are licensed healthcare professionals who may be registered nurses, registered dietitians, pharmacists, or have worked in any number of specialty areas. Diabetes Educators have the opportunity to earn two different credentials - Certified Diabetes Educator (CDE) or Board Certified Advanced Diabetes Educator (BC-ADM).
Diabetes education, also known as diabetes self-management training (DSMT) or diabetes self-management education (DSME), is defined as a collaborative process through which people with, or at risk for, diabetes gain the knowledge and skills needed to modify behavior and successfully self-manage the disease and its related conditions. The intervention aims to achieve optimal health status, better quality of life and reduce the need for costly health care.
Requirements
Education
• Clinical psychologist, registered nurse, occupational therapist, optometrist, pharmacist, physical therapist, physician (M.D. or D.O.), or podiatrist holding a current, active, unrestricted license from the United States or its territories, *or* • Dietitian holding active registration with the Commission on Dietetic Registration and license with the State of MN Board of Dietetics and Nutrition Practice, physician assistant holding active registration with the National Commission on Certification of Physician Assistants, *or* • Health care professional with a minimum of a master's degree in social work from a United States college or university accredited by a nationally recognized regional accrediting body. • After meeting this requirement, the educator can begin to accrue the minimum professional practice hours of DSME and continuing education hours needed prior to applying for the examination to become a Certified Diabetes Educator.
Experience
• Minimum of two (2) years of professional practice experience required. • Demonstrated competence in group teaching, presentation/public speaking skills for a diverse population. • Experience in curriculum design, program planning and evaluation is required • Evidence of valid cardiopulmonary resuscitation competence (AHA Guidelines).
Licensure/Certification
• Current license in one of the professional fields listed above under Education section -BH Standard • Board Certified Diabetes Educator (CDE) through the National Certification Board for Diabetes Educators (NCBDE) as outlined within two years from date of hire - BH Standard • Current Class D driver's license in the state of residency with acceptable driving record - BH Standard • BLS certificatio BH Standard
