Psychotherapist 2

About this position

A psychotherapist 2 assesses and treats the behavioral, psychological, and emotional aspects of the customer's needs; establishes with the customer appropriate goals for treatment, and reviews progress towards these goals. Experience: 1 to 2 years in clinical psychotherapy experience in a community based mental health clinic, a hospital group practice or independent practice. In collaboration with the leadership team, supports and guides the clinical model for the unit and/or clinic.
Psychotherapist 2 follows the customer from admit through discharge as the primary coordinator of care, including discharge planning. Direction of Others: Student interns; provides clinical supervision to LGSW Social Workers pursuing their LICSW and/or LPCC.

 

What we’ll expect from you

• Assessment and Diagnosis.
• Completes diagnostic assessments to diagnose, identify needs and establish goals under supervision by the LICSW and/or LP supervisor.
• Completes documentation that includes all required elements as defined by DHS using the co-sign process.
• Creates a treatment plan that included measurable goals and objectives, updates the treatment plan every 3 months and as needed reviewing the treatment plan with the LICSW and/or LP supervisor and the customer.
• Incorporates information from collateral resources when developing plan of care.
• The psychotherapist in the ED setting:
• Completes a diagnostic assessments to diagnose, identify needs and establish immediate risk of harm to self, risk to others and appropriate level of care.
• Completes documentation that includes all required elements as defined by DHS.
• Recommendations based on assessment and diagnosis to be communicated to customer, Physicians, nursing staff and parties assuming immediate care of customer with regard to disposition.
• Incorporates information from available collateral resources when developing plan of care.
• Determine appropriate level of care and facilitate admission to hospital, transferred to hospital create crisis plan for safe discharge, referral to mental health provider in an outpatient setting, referral for day treatment or crisis residence for further stabilization.
• Ability to work independently to make clinical recommendations to Emergency Department providers, physicians, psychiatrist, detox facility nurses and outpatient providers in regards to immediate safety concerns.
• Ability to provide information and complete documentation regarding health officer hold, 72 hour hold, guardianship/conservatorship, court holds and civil commitment process to ED providers, outpatient providers, customer and family/friends. Utilizing Risk Management as necessary to ensure customers' rights are being met.
• Perform ongoing assessments of customers while in department to re-evaluate level of care need or necessity for involuntary hold.
• Consultation with Emergency Department Psychiatrist, on-call Psychiatrist and clinical supervisors as needed for accurate diagnosis and treatment plan.
• Performs counseling and psychotherapy to assist clients/families in adjusting to and decreasing symptoms of mental illness.
• Provides follow-up psychotherapy sessions as determined based on client needs.
• Follows a treatment plan that identifies the type of therapy that would most benefit client and/or significant other/family.
• When goals are reached, discharge client and/or significant other/family from services.
• Communicates summary of care to those people the client has requested to be updated.
• Identifies and obtains necessary authorizations from insurance company(s) for initial and ongoing visits and stays within the guidelines of care authorized by the insurance company(s).
• Demonstrates a caring, positive regard for others by clarity of speech, use of understandable terminology and utilization of active listening skills.
• Respects and protects client's rights, including confidentiality.
• Acts as a client advocate and liaison between client and nursing, physician, and family to ensure that client self-determination is upheld.
• Employs therapeutic principles of communication, interviewing techniques, problem-solving and person in environment or systems theory.
• Uses knowledge of behavioral concepts such as anxiety, loss, conflict, anticipatory grief, and anger to assist client/family in coping, adapting and dealing constructively with feelings.
• Develops a safety/crisis plan with customer that includes follow-up crisis intervention techniques and resources.
• Completes a transport hold and refers client for further crisis assessment.
• Initiates 72 hour hold and communicates details of that hold to clients.
• Participate in weekly interdisciplinary team staffing.

Education

• Master's Degree or higher degree from an accredited university or college.   

Experience, Knowledge, Skills, Abilities 

• One to two years in clinical psychotherapy or direct mental health care in a clinical setting in community clinic, hospital group practice or independent practice. • Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.
• Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
• Performs assessment and diagnosis.
• Performs counseling and psychotherapy to assist clients/families in adjusting to mental illness.
• Provides psychosocial assessments of clients/families situations, including social, psychological, medical, emotional, financial, and other related factors.
• Performs counseling to assist clients/families in adjusting to illness.
• Assists client/family in utilizing relevant and appropriate community resources.
• Documents appropriate information in the client's medical record to provide direction to professional staff regarding plan of care.
• Maintains contact with community agencies to promote the exchange of information and facilitate use of these services on behalf of clients.
• Performs basic administrative tasks including statistical reporting and other related tasks.      

Licensure/Certification(s) Mandatory

• Current license as an Independent Clinical Social Worker who fulfills the requirements as set forth by the Minnesota Board of Social Work. - State Requirement
OR
• Current license as a Licensed Professional Clinical Counselor who fulfills the requirements as set forth by the Minnesota Board of Behavioral Health. - State Requirement
